I am writing to inform you of an important update from U.S. Citizenship and Immigration Services (USCIS) regarding Form I-693, Report of Medical Examination and Vaccination Record. 

USCIS has updated the form it uses for medical exams. It is important that your civil surgeon uses the newly updated form when they complete your medical exam to help ensure your case has the correct form and will be accepted by USCIS. Please ask your civil surgeon to use the 01/20/25 edition date of Form I-693.
